The room selection includes semi-private bedrooms along with newly remodeled private suites with cable television and lots of room for family visits. The setting is ideal for residents who need assistance with routine personal care activities and health services coordination by licensed nurses.

Well-planned community amenities like a beauty salon, restaurant-style dining rooms and a beautiful rehabilitative gym and courtyard provide an ideal environment for our residents to make friends and enjoy life.

Forest Grove Rehabilitation and Care Center offers a wide variety of services for seniors. Programs range from short-term, transitional care that bridges the gap between hospital and home to long-term placement for those unable to effectively mange in a more independent environment. All programs utilize a comprehensive assessment process and interdisciplinary approach to help residents achieve the highest possible level of independence and quality of life.

For residents who need assistance with activities of daily living, medication management, and periodic assessment and treatment by a professional staff, Forest Grove Rehabilitation and Care Center offers quality care with a personal touch. Twenty-four-hour care is prescribed by an attending physician and delivered by a team of licensed professional nurses and certified nursing assistants.

Rehabilitative care is available for residents recovering from hip, knee or cardiac surgeries, strokes, wound care, or other injuries and illnesses. Our experienced staff dedicate themselves seven days a week to help individuals transition from the hospital to home. Our unique Optimal Recovery program helps our residents recover quickly and regain the highest possible level of functionality.

Forest Grove Rehabilitation and Care Center is a contracted provider for Medicaid, Medicare and several managed care health plans.
