Porthaven Health Care Center embraces the Eden philosophy - creating an environment full of life – filling rooms with vibrant plants, therapeutic pets and visiting schoolchildren. At Porthaven, we believe that living is an essential part of the healing process so we work to make living here as enjoyable as possible.

Porthaven is a nursing and rehabilitation center that serves the northeast Portland neighborhoods of Rose City, Beaumont, Alameda, Hollywood and Irvington. Our room selection includes private and semi-private rooms.

Our rehabilitation programs are a wonderful solution for individuals who are stable and no longer require hospitalization, but do need intensive rehabilitation and recovery. Our residents find that our large gymnasium is ideal for improving range of motion and the center’s kitchen, laundry and bathroom are the perfect place for occupational therapy. These services are an important aspect of gaining full functionality.

For residents who need assistance with routine personal care activities, our center also offers health services coordination by licensed nurses, 24-hours a day.

Porthaven is a contracted provider for Medicaid, Medicare and several managed care health plans.
